転入届てんにゅうとどけ(tennyū-todoke)
Moving-in notification
Registers your address at the ward or city office after you land. It puts your address on the back of your residence card and starts your My Number, health-insurance and pension paperwork.
When you need it
Within 14 days of starting to live at your address after landing, and again within 14 days whenever you move. It applies to everyone who is issued a residence card (a stay of more than three months). Showing your residence card at the counter counts as the “notification of place of residence” the Immigration Control Act requires, so there is no separate immigration form. The office prints your address on the back of the card, creates your resident record (住民票) and mails your My Number notice to this address; National Health Insurance and National Pension enrolment are normally handled at the same visit. Forms and small details vary by municipality — the official page linked above explains the rule, your office explains its form.
Where to get it
The resident-registration counter (戸籍住民課 or 住民記録係 — the name varies) of the ward, city, town or village office for your new address. The form, usually titled 住民異動届 (“resident move notification”), is on the counter and you fill it in there; large-city offices often have an English version or a multilingual helper, so ask. Hotels, weekly mansions and other short-term lets generally cannot be registered as an address.
What to bring
- Residence card (在留カード) — or your passport if it carries the “residence card to be issued later” stamp from a smaller airport
- Passport
- If you are joining someone else's household: a document proving your relationship to the head of household (marriage or birth certificate) with a Japanese translation that names the translator
- A Japanese phone number if you already have one (daytime contact)
- If someone files for you: a power of attorney (委任状) and that person's own ID
Field by field
The usual boxes in the order they appear. Layouts vary by office and forms get revised, so treat this as a map, not a photocopy.

Common mistakes
- Filing before you have actually moved in — the office cannot accept a notification dated before the move.
- Writing your name in a different order, or abbreviated, from your residence card — it has to match the card exactly, middle names included.
- Leaving out the building name and room number — your My Number notice and every official letter is delivered to what is written here.
- Using your landing date as the move date when you stayed in a hotel first — the 14 days run from the day you settled at this address.
- Skipping the household-head box when you live alone — you are the 世帯主; write 本人 as the relationship.
- Leaving the office before the address is printed on the back of your residence card — banks and phone shops will ask for it.

Show this to the staff
Hand this to the counter staff. The Japanese says what you need and that you don't speak much Japanese; the translation below is for you.
EN · Excuse me, I don't speak much Japanese. I have just arrived in Japan and I'm here to file my moving-in notification (notification of place of residence). I have my residence card and passport. Can I also enrol in National Health Insurance and National Pension here today? Please show me how to fill in the form — in simple Japanese or English if possible.
